REGISTRATION
• REGISTRATION FEE of $100 per camp you register for must be either paid online with credit card when you submit your application, or a check/money order must be sent to the address below to secure a spot in camp.
• This fee will be credited toward tuition fee of the camp you attend.• Balance due at registration or you may mail it in ahead of time.
NOTE: If you choose to pay with credit card, there is a 4.9% processing fee. If you choose to pay by Check /Money Order, please submit the form online and immediately send the check in the mail to the address below. Please be sure to note the campers name.

(There will be a $25.00 fee for all returned checks and a $25.00 administration fee for any refunds issued before the May 1st refund deadline.)MEDICAL CERTIFICATE
Each camper must provide a current physical or a 2009-10 high school physical indicating you are able to participate in volleyball sport activities by the first day of camp. This physical may be returned upon request following the camp.
